Caitlin, when the participant is screened for SDoH and find her/him needs, what is the next step to address it? Do you refer them out? or do you provide services in house?

We use the HealthLeads SDoH screener; we continue to re-evaluate how to best deliver the screener and are hoping to make some edits to it this year (2021). We refer out most of the referrals (recognizing mental health, food insecurity and housing instability services are outside our scope); we utilize Unite Us/Unite Colorado as a referral network or we have referral champions in the community (our YMCA Association did become a food distribution hub due to the PHE). Our Community Liaison is able to track referrals and provide follow up.

New York State Medicaid covers the National DPP as a benefit, and CDC-recognized organizations in New York can apply to become a Medicaid provider to offer the program to Medicaid beneficiaries.

If we receive referral from an Advantage plan we don't have a contract with - we reach out to the health plan and attempt to negotiate a contract.

COVID success and survival stories have also helped us in discussing the program - we had a participant survive COVID and he gave a testimonial about how he credits NDPP/MDPP to surviving COVID
